ABOUT US
• The AlzCare office receives an initial referral request for services from a family or service agency, and, after an initial telephone screening, an in-home assessment is scheduled and completed by the designated home assessor.
• At that point, the Respite Coordinator selects a companion then schedules the initial visit. The family caregiver and the companion should be present, as this is an opportunity to meet all parties and discuss any special needs.
• Subsequent visits will be scheduled at this point, and families usually will continue to receive respite visits until the required care exceeds our service ability or the need for respite no longer exists.
